About the Role

We are seeking a dedicated and compassionate Nurse Practitioner or Physician Assistant (PA) to join our dynamic team at ReleviiMed. Key Responsibilities

-Conduct thorough assessments of assigned patients through physical examinations and medical history reviews.
-Collaborate with physicians, nursing staff, and other health professionals to develop and implement individualized care plans that address pain management, symptom control, and psychosocial needs.
-Monitor the effectiveness of prescribed treatments, adjusting care plans as needed.
-Educate patients and families on pain management strategies and treatment options, considering cultural differences and individual needs.
-Serve as a liaison between patients, families, and healthcare professionals to ensure high-quality care.
-Actively participate in discharge planning, multidisciplinary rounds, and family meetings.
-Collaborate with the Pain Management Medical Director to develop and implement pain education and multi-disciplinary protocols.
-Participate in research data collection, quality improvement initiatives, and care coordination for patients under consultation.

Qualifications

-Graduate of an accredited Advanced Nurse Practitioner Program or Physician Assistant Program.
-Acute Care Nurse Practitioner (NP) certification required.
-Licensed or license-eligible in the state of Texas.
-Prescriptive authority required.
-DEA Registration required or pending.
-Two or more years of experience as a healthcare provider preferred but negotiable.
-Pain management experience is desirable, though not required.
-Experience in wellness and functional medicine is welcome.
